Once you’ve received notification that you’ve secured a spot on our calendar, you’ll need to complete the following forms and submit payment to finalize your reservation. You may either complete the information on our Forms page, or click the buttons below to download PDF files of the forms, complete them, and email the signed forms to paula@casacolinatx.com.
